We will need this information to enable us to assist you depending on what we help you with.
We may also store your bank details where needed as this may be required in order for the DWP or other agencies to pay and benefits to you or to set up payment to ourselves.
Depending on the service we provide to you we may need to share information with:
- The DWP
- Your GP, any consultants or other health professionals
- Your bank
- Yours spouse or other family members (with your permission)
- Other Government Agencies
- Help For You colleagues.
How do We Store the Information and for How Long?
Any information will be kept in secure, locked cabinets in a secure property or on computers and phones protected by passcodes. We will hold your health information until our service to you is finished, at which point it will be destroyed or deleted. We will just keep basic details such as your name, address, and telephone number just in case you need us in the future. We will not hold copies of any completed application forms sent to, for example, the DWP or Local Council.
We will need your permission to hold and share your information as mentioned above. Should you choose our service then we will ask you to sign authority to give us this permission. You can withdraw or change this consent at any time and you have the right to request to see any personal information we keep.
